Key focus/scope

The Gloucestershire pilot will focus on disabled children and young people in transition. The initial group will be selected from a cohort of 30 people who are currently in receipt of social care individual budgets.

Overview of pilot and its purpose

The approach of the Gloucestershire pilot is to apply the personalisation model of social care, and adapt this for a personalised approach to health. The pilot aims to give people more choice and control over the health services and support they receive. NHS Gloucestershire is committed to working alongside local authority colleagues to promote good practice and to further develop joint working within personalisation.

Objectives of pilot

The pilot aims to:

  • establish a system that will enable recipients of personal health budgets to have choice and control and self-direct the support they need
  • create processes which are efficient and equitable
  • manage attitude to change, demand and expectation
  • determine the systems change and cost implications that arise from implementation
  • support people in shaping services, purchased through a personalisation process
