Key focus/scope

The Hull pilot will focus on people with long term conditions. A wide range of disorders will be covered, including neurological conditions. Hull also wants to integrate this work with their new palliative care (end of life) team who will identify people who want maximum choice and control in managing their own death.

Overview of pilot and its purpose

As a commissioner, the primary care trust is committed to ensuring the healthcare interventions it purchases enhance the wellbeing of the local population. As one of the first "Pathfinder" sites Hull, as a whole, and the City Healthcare Partnership in particular, are determined to provide services tailored around peoples' individual needs and circumstances rather than them having to fit into existing service processes. The operation of the pilot will allow City Healthcare Partnership (as a free standing social enterprise) to better understand how it can enhance the patient experience and promote better practice overall.

Objectives of pilot

The Hull pilot aims to:

  • promote further integrated working between health and social care
  • ensure people are able to shape care services to meet their needs
  • better understand the wider strategic and tactical challenges in establishing personal health budgets
  • determine the systems change and cost implications that arise from implementation
  • assess the benefits for patients in delivering improved care outcomes and experiences.
