Key focus/scope
The NHS Doncaster pilot will focus on people with continuing health care and mental health. The learning from the pilot will inform personalisation and choice in other service areas within the locality.
Overview of pilot and its purpose
In a move to commissioning for improved patient outcomes and satisfaction, NHS Doncaster is focusing onshifting the balance of choice and control to people, with a view to maintaining and improving health and wellbeing. Successful implementation of the project will:
- promote health through information and self care; people staying healthier for longer
- promote independence and achievement of other goals such as returning to work
- reduce health inequalities - greater equity by allowing personalisation within the NHS rather than through the marketplace
- improve patient satisfaction with care
- promote integration and partnership working
- stimulate genuine choices - those choices feeding into commissioning decisions.
Objectives of pilot
Working with its partners, NHS Doncaster is:
- developing an integrated service structure that records monitors and evaluates changes in health and well being outcomes
- developing integrated systems that incorporate a range of personal health/individual budget models to maximise the learning from partnership working
- ensuring patient/client engagement and involvement remains core to service development, while upholding the values of the organisations involved.
